A valid referral is required at  the time of  booking an appointment. 

Please contact the preferred practice location by phone or email to make an appointment. You will also receive a reminder SMS 2 days prior to confirm your appointment.

Your Initial Consultation

In preparation for your initial consultation, please arrive 15 minutes prior to appointment time to fill in necessary paperwork. 

Please bring with you your referral, Medicare card, private health insurance information if applicable

Please also bring any relevant scans, hearing tests, current medication list and blood test results to your appointment. 

Payment for consultation is made at the time of your appointment and eligible patients will receive a part rebate from Medicare following this.
